About the Job

Under the direction of the CARE Program Director, the CARE Transition Coordinator is responsible for helping participants navigate the transition to and from the Buchanan YMCA’s Center for Academic Re-Entry and Empowerment Program (C.A.R.E.) to a comprehensive or credit recovery high school and post-secondary opportunities. This support will include a combination of the following activities: case management support, training, informational workshops, and transition planning activities. The Transition Coordinator builds relationships with SFUSD schools and community CBOs to enhance resourcing for CARE students and families and partnerships for the CARE Buchanan program. The Transition Coordinator will work closely with our partners - San Francisco Unified School District and County Office of Education (SFUSD-COE) Teachers, Counselors, and Administrators. The collaboration between the YMCA and SFUSD-COE will focus on meeting the needs of youth by creating and supporting individual action plans towards transfer and graduation.

The Transition Coordinator will have a direct relationship with SFUSD-COE, the Department of Children Youth and their Families (DCYF), the Juvenile Probation Department (JPD), San Francisco Police Department (SFPD), and all other Buchanan C.A.R.E. partners. This position is a vital member of the Y’s Alternative Education Team; the candidate will be collaborating with partner sites and attending the multi-site monthly wellness team meetings. This position requires understanding the YMCA movement and will engage with cohorts and additional working groups to help advance the Y’s Mission.

Minimum Qualifications

  • 23 years or older
  • BA/BS or equivalent experience
  • 2+ years working with opportunity youth.
  • Must have a strong knowledge of San Francisco, specifically the Fillmore/Western Addition community, and the barriers young people have in relation to academic success and personal development.
  • Must be available for Monday-Friday during program hours with occasional nights and weekends as needed.
  • Previous Group Work, Supervision, Administration, and Community Collaboration experience
  • Experience with developing and managing budgets
  • Excellent leadership, relationship building, program promotion, organizational and verbal/written communication skills.
  • YMCA approved CPR and First Aid Certifications (may complete upon hire)
  • Fingerprint background check and TB clearance

Preferred Qualifications

  • 25 + years old
  • Master’s degree or equivalent social work experience
  • History of working within the Western Addition community or with community partners
  • Bilingual in Spanish and English
  • Experience with Training, Group Work and Strategic Planning
  • Case management credential
